ACTion Programme Book
Over the past eight years, the Aged Care Transition (ACTion) Programme has helped more than 70,000 patients in providing post-discharge care. In 2017, it was merged into the Hospital to Home programme. The client wanted a simple, professional and modern look. At the same time, the book should not look excessively expensive. It had to also convey authenticity and emotion.
Task
To publish a commemorative book that showcased the history, achievements, and the care coordinators involved in the programme.
